Fingerprinting Request: BY APPOINTMENT ONLY

Cottonwood Heights Police Department offers fingerprinting services to Cottonwood Heights residents or those employed in Cottonwood Heights only.

• Call 801-944-7100 to make an appointment (available Tuesdays and Thursdays)
• Bring photo identification
• Fill out this fingerprint request form
• Save the form to your computer and send it to the fingerprinting team (choose Fingerprinting from the dropdown)
• Pay $10 per fingerprint card

Only the person being fingerprinted will be allowed back with the fingerprint technician for fingerprinting services.
