Information and Packets
Business license applications and fee schedules can be obtained in each individual packet by visiting this pagePrint and complete the application.  The application must be submitted with the fees and any required verifications as needed.  You may submit the application by mail or bring it to the office.  You may also fax or email the information.  Please make checks payable to “Cottonwood Heights” or you may pay with cash or credit card (excluding American Express).
